Anyone know what gave a badge that shows up as an empty spot? I've been wracking my brain and I can't figure out what it is from.

Either ghost badge or one of the invisible ranks

the difference is so slight (3 millimeters or 0.11811 inches) i don't know how you are seeing it (space reserved for xx number of badges)
there are 25 mm to 1 inch

It is "Prestige 6", but unlike the other ranks it does not seem to have any specific styling.
